The Agency alleges that on May 15, 1992, Jim Mayo, present owner
and/or operator of a facility located in Henderson County andcommonly known to the Agency as Oquawka/Nayo, violated Sections21(p) (1) and 21(p) (5) of the Act.

there are two (2) such violations, the total penalty to beimposed is set at $1,000.00.
