AN AMENDED INDEX OF DOCUMENTS
Comes now the City of Madison, by and through its undersigned attorneys, and for its Motion
for Leave to File an Amended Index of Documents states
:

On April 12, 2007, the City of Madison filed its Certificate of Record on Appeal and
its Index of Documents.
2.
Subsequently, the City of Madison discovered that the Index of Documents contains
several misspelled names
.
3 .
The City of Madison seeks leave from the Pollution Control Board to file an
Amended Index of Documents to correct these errors
.
4.
The City of Madison's Amended Index of Documents is enclosed herewith
.
Wherefore, the City of Madison requests leave of Court to file its Amended Index of
