HEARING OFFICER ORDER
The Board intends to adopt a proposal for public comment in the above-captioned matter
on April 6, 2006 . Copies of the opinion and order will be forwarded during the week of April 10,
2006, to those whose names appear on the notice lists in the three dockets . The public comment
period in this matter will end on or about June 5, 2006
. This order explains what you must
do to remain on the notice list of this docket, and it offers you an option of how to receive
documents pertaining to Board actions in this matter.
A RESPONSE TO THIS ORDER IS REQUIRED BY APRIL 10, 2006 for
any person wishing to receive copies of the opinions and orders in this docket
and in any future dockets in these subject matters. A non-response will
result in removal of your name from the notice lists of these dockets and all
future dockets .
The Board plans to delete all names from the notice lists in this and future update
dockets unless there is an affirmative request to remain on the notice lists for a particular
sub ect matter
.
This is done to assure that only those persons who wish to receive copies of
Board opinions and orders in underground injection control, municipal solid waste landfill, and
hazardous waste proceedings continue to appear on the notice lists for these dockets . By
completing the reverse of this page and forwarding it as indicated by April 10, 2006, you
will assure that your name continues to appear on the notice lists in these update dockets as they
arise . If you do not respond with the appropriate information, your name will be removed
from the notice lists, and you will not continue to receive copies of Board opinions and
orders in these matters . The only persons who will continue to receive copies of Board
opinions and orders in this and future updates without submitting an affirmative response are the

AN AFFIRMATIVE RESPONSE IS NECESSARY if you are to continue to receive copies
of Board opinions and orders .
The Board will RETAIN YOUR PREFERENCES as to how you wish to
receive future Board documents THROUGH THE ENTIRETY OF THIS
PROCEEDING AND FOR ANY FUTURE DOCKETS IN THESE
SUBJECT MATTERS in which the opinion and order have an extreme
volume .
The texts of the opinion and the order in the above-captioned matter have an extreme
volume (over 1000 pages combined). The opinion will be over 100 printed pages, and the order
is over

printed pages. As a result, the Board is offering persons on the notice list their option
of how they are to receive copies of these documents . The options available for receiving these
documents are as electronic documents on a CD, as a paper copy of the opinion alone, as paper
copies of both the opinion and the order, or as an electronic document downloaded from the
Board's website. The Board is trying to avoid unnecessary costs of copying and forwarding these
documents and encourages receiving them in an electronic format on a CD
.
By completing the reverse of this page and forwarding it as indicated by April 10,
2006, you may select your preferred format and what you want to receive . The Board is mailing
a copy of this form to each person whose name appears on the UIC, RCRA Subtitle D, and
RCRA Subtitle C notice lists . The Board will mail the documents only upon request, except
that the Board will automatically forward paper copies of both documents to the primary contact
persons for the Joint Committee on Administrative Rules, the U .S. Environmental Protection
Agency Region 5, the Illinois Environmental Protection Agency, the Office of the Attorney
General, the Illinois Department of Natural Resources, and the Department of Commerce and
Community Affairs .
Please provide the necessary information in the appropriate spaces on the following page
