    The Petition for Variance in this matter is inadequate
    in that it
    fails to comply with the requirements of Rule 401 of the Board’s Pro-
    cedural Rules
    in that it fails to state the specific rule or regulation
    from which
    a variance is requested.
    Petitioner is ordered to file an Amended Petition within
    30
    days of
    the entry of this Order containing information as to the effluent being
    discharged and the status of negotiations with the Village of Addison.
    Failure to file the Amended Petition in the required time shall re-
    suit in dismissal.
