    the requirements of Rule 203(d),

    402, 404(f)
    and

    of Chapter
    3:
    Water Pollution of the Board’s Rules
    and Regulations.
    The Petition
    is inadequate for failure
    to specifically request or waive a hearing as required by
    Procedural Rule 401(b).
    The 90—day decision period shall
    begin with the filing of the Amended Petition.
    If this
    deficiency
    is not corrected within 45 days of the date of
    this Order,
    this matter will be
    subject
    to dismissal.
