SCA Services of Illinois,
Inc shall comply with the
Terms of Settlement described
in paragraphs
18-23 of
the Stipulation and Proposal for Settlement filed
with the Board on August
23,
1978;
7.
Permit No. l974—l3OP and Supplemental Permit No. 76-431
are hereby revoked, and the Agency shall
issue SCA
Services of Illinois,
Inc.
a new operating permit
effective on the date of this Order which shall incor-
porate the Terms of Settlement in the Stipulation and
Proposal for Settlement filed with the Board on August
23,
1978 and all other applicable
terms of previous
operating permits issued
to Respondent;

Within
30 days of the date of this Order Respondent
or SCA Services of Illinois,
Inc.
shall forward
the
sum of $10,000.00
by
certified check or money order
made payable to the State of Illinois
to:
