On October
8,
1979 Petitioner requested a variance from
Rule 305 of Chapter
6:
Public Water Supplies.
The Petition
is deficient in several respects.
First,
it fails to state
the amount of water pumped and the number and depth of wells
in Petitioner’s water supply as required by Procedural Rule
401(a)(3).
Second,
it fails to indicate why Petitioner is
unable to comply with Rule 305 as required by Procedural Rule
401(a)(5).
Third,
it fails
to describe any compliance prog-
ram as required by Procedural Rule 401(a)(6).
Fourth,
it
fails to assess the environmental impact that the variance
would impose on human,
plant and animal
life as required by
Procedural Rule 401(a)(7).
Fifth,
it fails
to state the in-
terim measures which would be undertaken by Petitioner should
contamination occur as
required by Procedural Rule 401(a)(10).
Sixth,
it fails to specifically request or waive a hearing as
required by Procedural Rule 401(b).
Consequently,
this matter is hereby dismissed.
