    On April

    1981 the City of Carmi filed a petition
    for variance from Rules
    602(b) and
    (c)
    of Chapter
    3:
    Water
    Pollution.
    That petition failed to either request or waive
    hearing
    as required by Procedural Rule 401(b).
    Unless that defect is cured by the filing of an amended
    petition within 45 days of the date of this Order, the petition
    will be subject to dismissal.
    IT IS SO ORDERED.
