    On January
    4, 1982 the
    Village of
    Ashland filed a petition
    for variance in this matter
    which the Board
    construes
    as re-
    questing variance from Rule
    602 of Chapter
    3:
    Water Pollution.
    That petition is deficient in numerous respects,
    Particularly,
    the following information is not included:
    1.
    The quantity and types of materials discharged
    (Procedural Rule 401(a)(4));
    2.
    Date showing present non—compliance
    (Procedural
    Rule 401(a)(5));
    3.
    A compliance plan (Procedural Rule 401(a)(6));
    4.
    A statement concerning
    the environmental
    impact
    (Procedural Rule
    401(a)(7));
    5.
    Permit status
    (Procedural
    Rule
    401(a)(8)); and
    6.
    A statement of measures
    to be
    taken to minimize
    environmental harm
    (Procedural Rule
    401(a)(10).
    Unless the Village files
    an amended petition within
    45 days
    of the date of this Order
    curing the
    above noted
    deficiencies, the petition
    will be
    subject to dismissal.
