an outdated
form still
distributed
by the Agency
but which
does not incor-
porate
many of the informational requirements contained in the
Boardbs Procedural Rule
401,,
The Clerk is therefore directed to serve
a copy of the
Procedural Rules upon Petitioner along with a copy of this Order,
and Petitioner is directed to file an amended petition in conform-
ance with Procedural Rule 401.
Petitioner’s attention is
specifically directed to Procedural Rule 401(b), which requires
that hearing either
a)
be specifically requested,
or b) be
unconditionally waived, and if so waived that the petition be
accompanied by an affidavit attesting to the truth of the facts
alleged.
If an amended petition curing these deficiencies

dismissal.
