    On
    June
    9,
    1982
    the
    Board
    received
    copies
    of
    two
    letters
    sent
    by
    Potomac
    to the Agency.
    In
    one
    letter,
    Potomac
    challenges
    the
    reclassification
    of
    its
    wastewater
    treatment
    plant
    from
    Group
    4
    to
    Group
    2.
    In
    the
    second,
    it
    petitions
    for
    variance
    from
    Rule
    1201
    of
    Chapter
    3:
    Water Pollution, to
    allow
    a
    Class 4 operator
    to
    operate
    its plant, noting
    that
    if
    the
    Agency’s
    plant
    reclassi-
    fication decision is reversed
    that
    variance would be unnecessary.
    The Board can take no action on
    the
    reclassification request.
    The
    Board
    construes the letter concerning the
    treatment
    plant
    operator
    as
    a petition
    for
    Board variance.
    This petition fails to
    contain much of the information required by the Board’ s
    Procedural
    Rule. 401.
    The Clerk is therefore directed to
    serve
    a
    copy
    of
    the
    Procedural Rules
    upon
    Petitioner along with a copy of
    this
    Order,
    and Petitioner is directed to file an
    amended petition
    in
    conformance with Procedural Rule 401.
    Petitioner’s attention is
    specifically directed to Procedural Rule 401(b), which requires
    that
    hearing either a) be specifically requested, or b) be
    unconditionally
    waived,
    and
    if
    so
    waived
    that
    the
    petition
    be
    accompanied by an affidavit attesting to
    the
    truth
    of the facts
    alleged.
    If an amended petition
    curing
    these deficiencies is not filed
    within 45 days of the date of this Order,
    this
    petition will be
    subject to dismissal.
