    ORDER OF THE BOARD

    The City of Farmington
    (City)
    filed
    a Petition for Variance
    on May 5,
    1983.
    That Petition did not include a request for
    or a waiver of hearing as required at 35 Iii.
    Adm. Code 104.124.
    The City
    is instructed to amend its Petition to either request or
    waive hearing.
    If hearing
    is waived,
    the City must submit
    affidavit or other proof in support of the material facts
    alleged in the Petition.
    Finally, the City is to so amend its Petition within fourteen
    days, or it will be subject to dismissal.
    IT IS SO ORDERED.
