    In its amended petition, filed October 17,
    1983, the City
    has waived the hearing which it had formerly requested,
    but has
    neglected to provide the affidavit required by
    35 Ill.
    Adm.
    Code
    104.124.
    If an amended petition curing this defect
    is not filed
    within 45 days of the date of this Order, this action will be
    subject
    to dismissal.
    (The Board would advise, but will not
    require, that the City file an amended petition incorporating
    into a single document its amended petition, the relevant portions
    of its original petition,
    and the affidavit required by this
    Order.)
