    On April
    19,
    1984 the Village of Atwood filed a Petition for
    Variance.
    That Petition is deficient in that it fails to specif-
    ically request or waive a hearing and fails to contain an affidavit
    or other proof
    in support of the facts contained in the Petition.
    Both are required by 35
    Ill. Adm.
    Code 104.124.
    Unless an amended
    petition
    is
    filed within 45 days curing the above noted defects
    this matter will be subject to dismissal.
