    ORDER OF THE BOARD
    (by J.
    Marlin):
    This matter comes before the Board upon an August 31, 1984
    motion entitled “request for extension of variance.”
    However,
    pursuant to 35 Ill. Adm.
    Code 104.123 this filing is a petition
    for variance extension and is hereby docketed as new case PCB
    84—146,
    thereby dismissing docket PCB 83—77.
    The petition is deficient because
    it
    (1) fails to request
    incorporation of the records Opinion and Order in PCB 83-77 and
    (2)
    fails to request a hearing or in the alternative,
    fails to
    waive hearing and submit the required affidavit.
    Unless the City of Tuscola files an amended petition curing
    the above-noted defects within 45 days of the date of this Order,
    the petition will be subject
    to dismissal.
