the Illinois Emergency Service and Disaster Agency’s emergency

response telephone number:

Notification pursuant to a statutory requirement;

Investigation by government authorities;

3)

Notification of

a release by a Federal

or State

permit

holsier when required by its permit;

Inventory efforts or

random or incidental

observation by government agencies or

the public;

Other sources.

If not reported previously,

a release should be promptly

reported to IESDA at telephone number ~

Any person

in charge of a vessel or

facility should immediately notify

IESDP. as soon as he

has knowledge of

a release (other than a State or

federally permitted release) of

a hazardous substance

from such vessel or facility in an amount equal

to or

greater than the reportable quantity determined pursuant

to Section 102(b) of CERCL~,as amended

(42 U.S.C.

9601

et seq.).

IESDA shall convey the notification

expeditiously to IEPA and other appropriate government

agencies.
