is deficient

it fails

to either waive

or request hearing, there

is no proof of service upon the

Illinois Environmental Protection Agency,

and the specific Board

rules from which variance

is being requested have not been

specified.

Furthermore,

the Board

requests that Franz submit

a

copy of NPDES Permit No.

IL0061859.

Unless

an amended petition curing the above—noted defects is

filed within 45 days of

the date of this order,

the petition will

be subject

to dismissal.
