    ORDER OF THE BOARD
    (by J.
    Marlin):
    This matter comes before the Pollution Control Board on an
    administrative citation filed by the Tazewell County Board of
    Health on March 20,
    1990.
    An examination of the copy of the administrative citation
    filed with this Board revealed the following deficiencies:
    1.
    The cover letter does not indicate a name and address
    in the space labelled “Respondent is being served at:”;
    and
    2.
    The first page of the administrative citation is
    missing.
    If the Tazewell County Board of Health does not file a copy
    of the administrative citation documents that cures these
    deficiencies within
    10 days of the date of this Order,
    the
    citation may be subject to dismissal.
    IT IS SO ORDERED.
