This
 matter
 comes
 before
 the
 Board
 on
 the
 July
 10,
 1991
petition for variance filed by the petitioner.
 The petition
 is
deficient in that it does not comply with the Board’s procedural
rules
 for
 form
 and
 service
 of
 filings
 with
 the
 Board.
Specifically, the petition is not signed by the attorney (35 Ill.
Adm. Code 101.103(g))
 and no proof of service was filed with the
petition
 (35 Ill. Adm.
 Code 101.141 and
 101.143).
 In addition,
the petitioner should provide the results of sample analyses
 for
the contaminants in question from January 1989 to the present
 (35
Ill.
 Adm. Code 104.121(e)).
If an amended petition curing these deficiencies is not filed
within 45 days of the date of this Order,
 this petition will be
subject to
 dismissal.
 The filing
 of
 an
 amended petition will
restart the Board’s decision deadline pursuant to Section 40 of the
Act.
