    ORDER OF THE BOARD
    (by C.A. Manning):
    On June 1,
    1994, Robert 3. Sheehy
    & Sons Funeral Home
    (Sheehy)
    filed a petition for review of an Illinois Environmental
    Protection Agency’s (Agency)
    final reimbursement determination
    from the Underground Storage Tank Fund.
    The Agency issued a
    final reimbursement decision on April
    1,
    1994,
    accompanied by
    Attachment A which contains the stated reasons for deductions.
    The Board on June 30,
    1994 found Sheehy’s petition to be
    deficient because it failed to provide the necessary Agency’s
    invoice voucher and ordered it to file an amended petition within
    45 days of that order.
    Sheehy has not filed an amended petition within the 45 day
    time period specified by the Board.
    Accordingly, the Board
    hereby dismisses and closes the docket in this matter.
