    ORDER OF THE BOARD
    (by R. C. Flemal):
    On May 31,
    1994,
    Richard E.
    Gaunt,
    Sr. filed a petition for
    review of an Illinois Environmental Protection Agency (“Agency”)
    final reimbursement determination from the Underground Storage
    Tank Fund.
    The Agency issued a final reimbursement decision on
    April 19,
    1994, accompanied by Attachment A which contains the
    stated reasons for deductions.
    The final determination concerns
    Richard E. Gaunt,
    Sr.’s site located at 2354 Washington Road,
    Washington, Tazewell County, Illinois.
    The petition fails to provide the Board with attachment A to
    the Agency’s invoice voucher.
    If an amended petition curing this
    deficiency is not filed within 45 days of the date of this order,
    this petition will be subject to dismissal.
    The filing of an
    amended petition will restart the Board’s decision timeclock.
