of Procedural Rule 401,

proceeding will be dismissed.

A clear and unconditional waiver of hearing or,
alternatively,
averred—to facts sufficient to enable the
Board
to rule on the petition.

2.
A detailed description
of proposed equipment and
the proposed methods
of control
to be undertaken to achieve
full compliance with Rule 304(c)(1)(B)
of Chapter
6,
including
a time schedule for implementation of
full compliance.

3.
A statement of the measures to be undertaken during
the period of the variance to minimize the impact of the dis-
charge of
contaminants.
Rule 401(a)(10).
4.
A concise factual statement
of the reasons that
compliance with Rule 304(c)(1)(B)
of Chapter 6 would impose
an arbitrary or unreasonable hardship upon Petitioner.
