    ORDER OF THE BOARD (by C.A. Manning):
     
    On May 11, 2001, the Illinois Environmental Protection Agency filed an administrative citation
    with the Board against Alan Smith (Smith), pursuant to Section 31.1(b) of the Environmental Protection
    Act (Act) (415 ILCS 31.1(b) (2000)). The administrative citation alleges that Smith caused or allowed
    the open dumping of waste in a manner that resulted in open burning. This activity was in alleged
    violation of Sections 21(p)(3) of the Act (415 ILCS 5/21 (p)(3) (2000)). The alleged violation
    occurred at a site located in Urbana, Champaign County, Illinois.
     
    A petition for review must contain certain information and documents as specified in Parts 101
    and 108 of the Board’s procedural rules (35 Ill. Adm. Code 101, 108). The petition must contain a
    notice of filing, certificate of service, and notice of appearance. Smith failed to include any of these
    documents.
     
    The Board grants Smith leave to file an amended petition for review. If an amended petition
    curing these deficiencies is not filed within 35 days of the date of this order, that is on or before July 26,
    2001, a default order may be entered.
