    This matter
    is before the Board on a November
    14,
    1989
    petition for extension of variance filed by the City of East
    Moline
    (East Moline).
    This petition seeks
    an extension until
    September
    30, 1991
    of the variance granted January 21,
    1988
    in
    POE 87—128 from
    35 Ill.
    Adm.
    Code 602.105(a)
    and 602.106(b)
    as
    they relate to trimalomethanes.
    The Board contrues
    this petition as a new petition for
    variance.
    This petition
    is deficient
    in
    that
    it:
    1.
    Fails to expressly
    request
    or waive
    a public hearing,
    and
    2.
    Does not include an affidavit executed by
    a knowledgeable
    person verifying that the facts asserted are
    true.
    3.
    Paragraph
    7
    refers to high bids rejected by the City,
    but
    fails
    to state the amounts of these bids.
    Unless an amended petition curing
    these deficiencies
    is
    filed within
    45 days of the date of this Order,
    this petition
    will be subject
    to dismissal.
    The date of filing for the purposes
    of Section
    38(a)
    of the
    Environmental Protection Act,
    Ill.
    Rev.
    Stat.
    ch.
    ll1~-, par.
    1038(a), as
    it relates
    to the time for Board action,
    shall
    be the
    date on which East Moline files
    its amendment.
