    Petitioner is
    to
    supply
    the
    Board with the date or dates
    Duncan First Home Additions
    were built;
    the date Restricted
    Status was imposed
    on the
    City;
    the feasibility and cost of up-
    grading or repairing the
    existing form
    of treatment,
    supplementing
    the cost figures already provided for a new private sewage disposal
    system; the feasibility
    and costs of
    installing water saving devices
    at the homes
    located
    at
    Duncan First Home Addition and figures
    estimating the discharge reduction which could be anticipated should
    they be installed;
    the locations and sizes of the sewer extension
    and the existing sewer to be interconnected.
    The Duncan First Home Addition
    is joined as necessary party.
    Petitioner is ordered to serve the homeowners as individuals or as
    an Association in Accordance
    with
    35
    Iii.
    Adm.
    Code 103.123.
    Failure of Petitioner to amend
    the
    Petition as filed with the
    foregoing within 45 days
    of the
    date of this Order shall render the
    Petition subject to
    dismissal
    for
    inadequacy
    (35 Ill. Mm. Code
    104.125).
