    a petition requesting a variance from all of the Chapter

    Water Pollution Rules and Regulations for a
    period
    of at least
    five years.
    The petition is deficient in a number of respects.
    It does not contain sufficient facts
    to establish that Petitioner
    needs a variance from all of Chapter
    3,
    Furthermore, Petitioner~s
    operation appears
    to be subject to Chapter
    4:
    Mine Related Pollu-
    tion,
    Procedural Rule 401(a) (1)
    requires specific identification
    of the particular provisions
    of the regulations.
    Procedural
    Rules
    401(a) (4)
    and
    (5)
    recmire identification of the quantity and types
    of materials discharged and the nature and extent of the present
    failure
    to meet numerical standards.
    Procedural Rule 401(a) (10)
    requires
    interim discharge
    limitations.
    The petition will be
    subject to dismissal unless these deficiencies are corrected
    within forty-five days of the date of this Order.
    The ninety
    day decision period will commence with the filing of an amended
    petition.
