    ORDER OF THE BOARD
    (by Dr. Satchell):
    On January 28, 1980 General Electric Company
    (GE)
    filed a
    variance petition requesting relief from manifest requirements
    of Rules
    301 and 302 of Chapter 9:
    Special Waste Hauling Requ-
    lations
    Since the petition fails
    to specifically request or
    waive a hearing it is deficient under Procedural Rule 401(b),
    Furthermore,
    there is no compliance olan as required by Pro-
    cedural Rule 401(a) (6),
    According to the petition GE manufac-
    tures lamps for photographic use at its Mattoon Lamp Plant,
    Coles County.
    It generates one to five loads of borosilicate
    glass scraps per day which are landfilled,
    It has obtained
    a
    special waste permit but seeks
    a variance from the manifest
    requirements.
    The petition does not contain a detailed esti-
    mate of the cost of compliance which may be necessary to sup-
    port an allegation of arbitrary and unreasonable hardship.
    GE
    should investigate the cost of manifest
    forms
    with the necessary
    information pre-printed and savings which may be achieved by
    using a copy of the manifest for internal record keeping as was
    suggested by the Board in its Opinion of March 15, 1979

    Unless additional information
    is received within forty-
    five days of the date of this Order, the petition will be sub-
    ject
    to
    dismissal.
