13 and 14.

The

Petition is insufficient in two respects~

First,

it fails

to specifically request or waive a hearing as required by

Procedural

Rule 401(b),

Second,

it falls to provide proof

of service upon all of the parties to PCB 70-7,

12,

13 and 14.

If these inadequacies are not corrected within

45 days

of the date of this Order,

this matter will be subject to

dismissal.

The statutory 90-day decision period shall

begin to run on the date an

Amended Petition

is filed.
