    The Board construes the document filed herein on December

    as a Variance Petition.
    However, Petitioner has failed to
    indicate whether a hearing is requested or waived,
    as required
    by Procedural Rule 401(b).
    Allied Chemical Corporation shall amend its Petition within
    35 days of the date of this Order to provide the necessary infor-
    mation.
    The 90-day decision period shall commence with the filing
    of the Amended Petition.
    Failure to file an Amended Petition shall
    subject this petition to dismissal due to inadequacy.
