On October 26, 1977 Petitioner requested an extension of

a prior variance from the effluent limitation for cyanide con-

tained in Rule 408(a)

of Chapter

3:

Water Pollution of the

Board’s Rules and Regulations.

The petition is inadequate for

failure to specifically request or waive a hearing as required

by Procedural Rule 401(b).

The pethtion states that this is

Petitioner’s

fifth request for a variance from the cyanide

standard, but does not state the docket numbers of these prior

proceedings or whether their records should be incorporated

into this proceeding.

If these deficiencies are not corrected within

45 days

of the date of this Order,

this matter will be subject to

dismissal.
