    ORDER
    It
    is the Order of the Pollution Control Board that Petitioner
    is hereby granted a variance from the requirements of Rules 203(f),
    402 and 404 of Chapter
    3: Water Pollution of the Boards
    Rules and
    Regulations for a period of one year from the date of this Order
    subject to the following conditions:
    a)
    Maryville Colonial Nursing Home shall apply for and
    obtain all necessary permIts from the Agency in order to com-
    plete its selected alternative.
    b)
    Maryville Colonial Nursing Home shall take all nec-
    essary steps to ensure that the nature of the effluent from
    its treatment plant does not markedly change from that pre-
    sently discharged.
    c)
    ‘Within 35 days of the Board~sfinal Order herein,
    Petitioner shall execute and forward to the:
    Variance Section
    Division of Water Pollution Control
    2200 Churchill Road
    Springfield, Illinois 62706
    Attention:
    Roger Callaway
    a Certification of Acceptance and agreement to be bound to
    all terms and conditions of the variance.
    The form of said
    Certification shall be as follows:
