    choice of or waiver of hearing required under Procedural Rule
    401(b) has been expressed.
    In addition, the condition of the
    receiving stream (odors,
    if any; sludge deposits; algae;
    etc.)
    is not given in any detail.
    Petitioner shall file with the Board and Agency an Amended
    Petition providing the required information within 45 days
    of the date of this Order.
    Failure to timely
    file
    the Amended
    Petition shall render the cause liable to dismissal for
    inadequacy.
    The 90—day decision period set by statute shall
    run from the date of filing the Amended Petition.
