    (Act)
    by the Illinois

    A copy of that

    Service of the
Administrative Citation was made upon Renaissance Restoration on

    The Agency alleges

    It
    is hereby ordered
    that,
    unless the penalty has already
    been paid,
    within

    Restoration shall, by certified check
    or money order payable
