    the Board issued an Order concerning
    this variance petition directing that a request or waiver of
    hearing be submitted and that if hearing is waived that an
    affidavit be included averring that the facts stated in its
    August
    11,
    1980, petition are true.
    The Board received such an affidavit on September 25,
    1980,
    but did not receive either a request or waiver of hearing pursuant
    to Procedural Rule 401(b).
    If the petition is not amended to cure
    this defect within 45 days of the date of this Order,
    it may be
    subject to dismissal.
