This petition for variance, filed June 29,

fails to
provide much of the information required by Procedural Rule 401.
More specifically, the petition does not provide the barium
content of each well,
and does not provide information concerning
the distribution system

Possible compliance methods
and timetables, with their attendant costs, have not been prescn-
ted

9).
Finally,
the petition fails to specifically
request or waive a hearing
(if hearing
is waived, the petitioner
must submit an affidavit attesting to the truth of the facts
alleged in the petition)
401(b).
If an amended petition correcting
these deficiencies
is
riot
filed within 45 days of the date of this Order,
this petition will
be subject to dismissal.
