    COMES NOW, Respondent,
    T.L. Trucking Foodliner, and in support oftheir Application,
    states as follows:
    1.
    The Illinois Pollution Control Board ordered numerous remedies
    be completed by
    Respondent in its Order dated August 7, 2003.
    2.
    All items to be completed within 40 days of the ruling have been completed.
    3.
    Items b, c and d, which were to be completed within 90 days ofthe ruling, have been
    completed.
    4.
    Item a, namely installation ofan interlock device, has not yet been completed.
    5.
    Respondent has hired an expert to complete the interlock device.
    6.
    Attached hereto as Exhibit A is a letterfrom AppliedMechanical Technology, LLC,
    indicating that due to back order ofsomeofthe parts and the necessity oftesting before installation,
    these interlock doors cannotbe completed until approximately December
    12, 2003.
    7.
    Given that all other matters set out by the Board were met in a timely fashion,
    and
    delay
    in
    completion
    of this
    item
    is
    due
    to
    factors
    outside
    Respondent’s
    control,
    Respondent
    respectfully requests that they be allowed until December 12, 2003, to finish the interlockoverhead
    door system.
    WHEREFORE,
    Respondent respectfully requests that the Board enter an Order allowing
    extension ofthedeadline to its requiredrulingunder paragraph4(a), namely, installationofinterlock
    doors, until December
    12, 2003.
