    ORDER OF THE BOARD (by C.A. Manning):
    On March 1, 2001, the Board granted the petitioner an extension of the time period for
    filing an appeal of a denial of high priority action plan and budget until May 15, 2001.

    April 11, 2001, the petitioner filed an appeal.
    This petition is deficient in two respects. First, the petitioner failed to include proof of
    service of the petition for review on the respondent. See 35 Ill. Adm. Code 101.304(d)
    (2001). Next, this petition appears to have been filed by a company employee. Section
    101.400(a)(2) provides any person other than individuals must appear through an attorney-at-
    law. See 35 Ill. Adm. Code 101.400(a)(2) (2001). The petitioner failed to include an
    appearance.
    Accordingly, the petitioner must file an amended petition showing proof of service on
    Agency and an appearance by an attorney.
    If an amended petition curing these deficiencies is not filed within 35 days of the date of
    this order, that is on or before May 24, 2001, this petition will be subject to dismissal.
