    the City
    of Casey
    (City)
    filed a
    Petition for Variance from Rule 305 of Chapter
    3:
    Water
    Pollution.
    Although the City purports to waive hearing,
    that waiver was not accompanied by an affidavit swearing to
    the truth of the facts alleged,
    as required by Procedural
    Rule 401(b).
    Further, the petition contains no allegations
    concerning the environmental impact of the request.
    Unless these defects are cured by the filing of an ar~end~d
    petition within 45 days of the date of this Order,
    the petition
    will he subject to dismissal.
