I, Steven C. Townsend, being first duly sworn and under oath state as follows:
1. I am a field inspector employed by the Division ofLand Pollution Control ofthe
Illinois Environmental Protection Agency (“Agency”) and have been so employed at all times
pertinent hereto.

On January 9, 2004, at approximately 10:25 A.M., I accompanied Sangamon
County Sheriffs Officer Daniel J. Hawk Hoffman, Sr., who personally hand-delivered and
served a copy ofthe attached Administrative Citation on Billie Landers, Registered Agent for
Equipping the Saints Ministry International, Inc. at their Route 104 W. location in Auburn,
Illinois. Further affiant sayeth not.
