    ORDER OF THE BOARD (by J.D. Dumelle):
    Triis matter comes before the Board upon Petitioner’s
    Petition For Variance filed November 24, 1987. The Petition is
    insufficient because it fails to comply with the requirements of
    35 Ill. Adm. Code 104.121. In specific, the Petition fails to
    include a compliance program, fails to state the time iengtn of
    the requested variance, fails to demonstrate service on
    trie
    Illinois Environmental Protection Agency and fails to demand or
    waive hearing pursuant to 35 Ill. Adm. Code Section 134.124.
    The Board hereby orders Petitioner to file an amended
    petition curing the above—noted defects. The amended petition
    shall be filed with the Board within 45 days of tne date of this
    Order, or this matter wiil be subject to dismissal.
