    ORDER OF THE BOARD (by J.D. Dumelle):
    On February 24, 1982 the City of McHenry (City) filed
    a petition for variance from the Operator Certification re-
    quirements of Chapter 3: Water Pollution, which purports to
    waive hearing but fails to include an affidavit attesting to
    the truth of the facts alleged as required by Procedural
    Rule 401(b). Unless the City files an amended petition within
    45 days of the date of this Order curing this defect, the petition
    will be subject to dismissal.
    IT IS SO ORDERED.
