    ORDER OF THE BOARD (by

    This matter comes before the Board upon a November
    1,
    1984,
    petition to extend variance filed on behalf of the City of
    Farmington.
    That petition
    is insufficient
    in numerous respects.
    The Board notes that, pursuant to
    35
    Ill. Adm. Code 104,123,a
    petition to extend variance must meet all of the informational
    requirements of an original variance petition, including a waiver
    of or request for hearing pursuant to
    35 Ill.
    Adm. Code 104,124,
    Many of these deficiencies could be cured through the incorpora-
    tion by reference of the record, opinion and order in PCB B3-63
    (the original variance proceeding).
    However, such incorporation
    must be specifically requested and any information which has
    changed since that time should be specifically noted.
    Unless the City files an amended petition within 45 days of
    the date of this Order during the above—noted deficiencies, the
    petition will be subject to dismissal,
