    On February
    13,
    1980 the City of East Peoria filed a
    variance petition which
    is deficient in several respects.
    It
    fails
    to req~uestor waive a hearing as required by Procedural
    Rule 401(b).
    It does not identify Board regulations from which
    the variance is sought as required by Procedural Rule 401(a) (1).
    The point of discharge and receiving waterway are not identified
    as required by Procedural Rule 401(a) (4).
    Unless these defi~
    ciencies are corrected within forty—five days of the date of
    th~sOrder,
    the petition will be subject to dismissal.
