This matter comes before the Board
 upon
 a December 16,
 1986
petition for variance filed
 on behalf of the City of South
Beloit.
 This petition
 is deficient
 in that
 it
fails
 to conform
to
 the requirements of
 35
 Ill.
 Adrn. Code 104.121—104.124.
Specifically,
 the City should specify from which particular
provision of
 the
 Board’s regulations
 the City seeks variance.
Also,
 the City should provide
 a more detailed explanation of the
proposed compliance strategy and
 a detailed assessment of the
environmental impact of the variance should
 it be granted.
 The
City should also submit
 a copy of the NPDES permit
 and should
include either
 a request
 for
 a hearing
 on its petition or
 a
statement waiving hearing accompanied
 by an
 affidavit attesting
to the truth of the facts asserted
 in the variance petition.
Unless this information
 is filed within 45 days of the date
of this Order this matter will
 be subject to dismissal.
